How to Complete a Survey

Community Survey | We Need Your Input

Surveys will be landing in mailboxes the week of November 11 and are due Monday, December 4.

How do I take the survey?

The survey can be taken online using your unique code provided or return by mail.  

Online:

1.    Go to the survey website: http://www.spfeedback.com/  
2.    Enter the Survey Access Number printed on the survey

Paper: 

If you do not have Internet access, please return the survey to any school office or mail to:

Additional Surveys - Did not receive a survey:  

The Survey Access Number can be used only once. To obtain additional surveys for other adults in your household, please call the District Office at 715.294.4140.
 
Results of the survey will be presented on Wednesday, December 11 during a Board of Education meeting. After the presentation, results will also be available on the District website.
